INSURANCE INFORMATION:

Our practice is going through significant changes in our participation with insurance companies. We have terminated our participation with several major insurance companies. Please call our office if you have questions related to coverage and/or participation.

If we do not participate with your health plan, you will be responsible for payments. we will provide you with a financial statement that you can submit to your insurance company for reimbursement. Obtaining a reimbursement for out or network providers is subject to your policy issued by your insurance. We cannot guarantee payments by your insurance.
